Students who are aware they will be absent from school due to non-school related activities, religious observances, or at the request of parent/guardian are asked to complete this Pre-Arranged Absence form. This form MUST be submitted at least 5 DAYS prior to the scheduled absence.
Please use this form for the following reasons:
-You know you will be absent for 3 or more consecutive days.
-If the absence is not 3 or more days, please submit using our Report Student Absence Form
Any student who is absent for an extended period of time must make up all the work missed as determined by their teacher. Once this form is submitted, you will receive an approval from your administrator. At that point, it is the student's responsibility to inform all teachers via email of their absence and collect their make-up work.
