Emergency contact information is required for every student. DHS & PDE forms separate emergency contacts from approved pick up contacts. In most cases, you will select the same people, but you have the ability to designate different people if you wish.
If you do not have the information requested for any field, you must still complete the field. You can use NA for any text fields, select Pennsylvania for the state, and enter all one's for numeric fields.
You can edit this form at any time by keeping a copy of the email confirmation you receive. At the bottom of the email you will see a link to "edit submission."
These forms are periodically updated, so we will also be sending you requests to update. The email usually comes from Jotform.
Your child will not be able to begin school until we have this form completed. We appreciate your cooperation and understanding.
