I would like to purchase a Worship & Dance t-shirt for my child(ren) to keep. Cost is $25. Please note that the same t-shirts will be provided every Family Sunday for children to borrow and return should you not choose to purchase one.
Weekend Practices and Communion Sundays:*

I am aware that practices for this program are scheduled for the weekend of the scheduled Family Sunday.I understand that my child(ren) must be able to attend all practices the weekend of, to participate in leading worship at the Family Service.I agree that my commitment as guardian is to help my child(ren) be on time for scheduled practices and scheduled service days. I will do my best to help my child practice at home using the practice videos that will be provided to me by the church.I understand that my child(ren) is committing to practicing at home throughout the month independently and if they don't demonstrate having knowledge and sufficient understanding of the actions they will have the opportunity to participate in the next Family Sunday.
Planning Center Scheduling:*

Scheduling for this program will be done through the Planning Center software/app. By checking this box, I agree to learn and use Planning Center as it relates to this program (information and tutorials for Planning Center will be sent via confirmation email upon submitting this registration form).
Video/Picture Consent:*

This program is run during the Family Service on Sundays, which is live streamed on our website each month. By checking this box, I give the Family Christian Centre permission to take videos and pictures of my child(ren) for live stream and promotional purposes.
