MEMBERSHIP FREEZE REQUEST
You must allow 5 business days for any freeze to go into effect. Any billing due between now and 5 business days from now will be processed.
The following rules apply to membership freezes:
1. You may freeze your membership one time per calendar year for any reason for a period of 1 to 6 months. You will not be billed your normal membership fees during the freeze period, but will incur a maintenance fee of $3/month.
2. You may freeze your membership for $0 maintenance fee for medical reasons or temporary relocation. To do so, do not submit this form. Please email your freeze request with a medical note or proof of relocation (or proof of travel) to your home club. You may also bring your documentation to either Gold's Gym location.

If you're choosing option 1 above and doing a maintenance freeze for $3 per month, please provide your information below and click SUBMIT. You will also receive a copy of this request via email for your records.
Please note that if your annual fee is due during your freeze period, it will be billed accordingly on the scheduled date.
