MDD is a private practice office that is here to help our clients reach their wellness goals. We submit claims as a courtesy to our clients. Ultimately, the client is responsible for any balances due.

MDD Fee for Service charges for Non-Insured/Non-Covered claims: This is the maximum you could/would be charged by MDD if your plan denies your claim:

First Appointment: $150. Second (Meal Plan) Appointment: $115. Follow-Ups: $75.

Most, not all, of our clients have some type of nutrition counseling through their insurance. Most are covered, co-pays are common, and denials are rare, but they do occur.

MDD Appointment Policy

To schedule an appointment, we require a valid credit card or otherwise acceptable payment method to be kept on file to pay for any incurred charges. MDD reserves right on this policy.

At MDD, Inc. our Registered Dietitians see clients by appointment only. A last-minute cancellation or no show is time another client could have used. MDD requires a minimum 24-hour notice prior to your appointment to avoid a $75 fee.

Missed Appointment Charge: Your card on file will be charged $75 for a no-show or last-minute cancellation.

MDD believes this policy is unfortunate but necessary.
