• Baptism Application Form

    Stage 2 of 2
  • Before you begin

Parents complete a compulsory online course to prepare for baptism, called Belonging (Ascension Press with Fr Mike Schmitz).
This must be completed before booking a baptism.
You need a copy of the certificate (which you will receive after completing the course) to complete this form.

You also need to attend Sunday Mass with your child the weekend before your baptism to be welcomed before the community.

A brief monthly meeting is held on the second Wednesday of the month at 7pm in the Parish Church to meet you, to answer any questions and walk you through the liturgy.
This meeting must be attended before completing this form.

Read the Baptism Preparation document before continuing.

Please complete one form per child.
