ALL SUBMISSIONS TO THE USDA ARE COMPLETED DIGITALLY. WE MUST RECEIVE YOUR COMPLETED TRAVEL INFORMATION AND RABIES CERTIFICATES AT LEAST 24 HOURS PRIOR TO YOUR APPOINTMENT. FAILURE TO DO SO MAY RESULT IN THE CANCELLATION OF YOUR APPOINTMENT:
• RABIES CERTIFICATE(s) MUST INCLUDE: Signature of veterinarian, microchip number, the vaccine
manufacturer, product name, serial or lot number, Lot/serial expiration, date administered, and date vaccine
expires. If your pet’s rabies vaccine occurred more than one year ago, please send the last TWO rabies vaccine certificates.
ADDITIONAL DOCUMENTS (COUNTRY DEPENDENT) THAT MAY BE REQUIRED:
• FAVN RABIES TITER REPORT
• FEDEX LABEL (PRIORITY OVERNIGHT) FOR THE DELIVERY OF YOUR OFFICIAL DOCUMENTS
• SUPPORTING DOCUMENTS (IMPORT PERMITS, ETC.)
We strongly encourage you to email us (info@UAHNYC.com) copies of your travel documentation (rabies certificates, etc.) so that we can confirm that the necessary requirements of your destination country will be met. The accuracy and timing of your paperwork, as well as any required treatments, vaccines, and/or tests, are crucial to the USDA’s official endorsement approvals. We will not alter or correct inaccurate documentation from another facility and we are not responsible for inaccurate, missing, or incomplete documentation.
