Direct Support Professional of the Year Nomination
Each nomination must include a summary response to each of the criteria below:
Promoting Person-Centered Plans: How does the nominee champion the person/people they support and assist them in meeting goals?
Relationships: How does the nominee support the person/people in creating and maintaining friendships and relationships beyond the DSP?
Leadership: How has the nominee demonstrated teamwork and leadership?
Innovation: How does the nominee think “outside the box” to solve challenges or improve the quality of life of the person supported?
Additional Accomplishments: If the nominee has done something else that does not quite fit into these categories, we will also take that into consideration.
