1) This form MUST have a supporting document attached at time of lodgement.
2) 8% SST and credit card merchant/ banking processing fees will not be refunded. An admin fee of RM10.00 will be charged for refund processing.
3) In the circumstance that a student is hospitalised, a refund on unused lesson WILL BE CONSIDERED but NOT GUARANTEED, upon presentation of doctor's letter or medical documents.
4) Any incomplete and/or incorrect information and/or an improperly declared form will not be entertained and a refund will not be processed.
5) All refund requests are subject to management's approval and will take up to 21 working days to process.
6) Slot will immediately release once the form submitted, and does not guarantee the same schedule or swim teacher when reenroll.
7) Reassessment may be required for returning students with more than 3 months break.
8) Registration fee will be collected for returning students.
