I understand and agree that Memories PhotoBooth by Le’Cam is contracted for the time stated in this questionnaire. Memories PhotoBooth by Le’Cam is not responsible for your event starting late or no guest showing up. If you want Memories PhotoBooth by Le’Cam to stay additional time after what is stated, there will be an additional cost.
I understand and agree that additional time for the photo booth is soley at the discretion of our attendant for an additional charge.
I understand and agree that we need accessibility for our equipment into your event. There are certain booths in our inventory that cannot be transported dvia stairs and an elevator is necessary to set up some of our photo booths.
I understand and agree that if you or any of your guests damage any of the equipment brought by Memories PhotoBooth by Le’Cam. You are 100% responsible for the cost of the damaged equipment, along with the balance for the event.
I understand and agree that if the Memories PhotoBooth by Le’Cam attendant experiences any kind of misconduct, disrespect, racial or sexually unacceptable behavior, Memories PhotoBooth by Le’Cam will let you know of this situation and if it happens a second time, our services will be stopped immediately with no monetary refund for services.
I understand and agree that the balance for your event is due no later than 1 day before our services begin (with cash, credit, or electronic payment). Failure to have the payment ready will result in a $50 extra charge.
I understand and agree that Memories PhotoBooth by Le’Cam is not responsible for any mistakes you have made in the questionnaire. Please make sure you double check all your answers to avoid any issues or miscommunication.
