ASSOCIATED CATHOLIC CEMETERIES

SUBJECT TO THE RULES AND REGULATIONS OF ASSOCIATED CATHOLIC CEMETERIES NOW IN EFFECT OR HEREAFTER ENACTED

*In accordance with RCW68.50.160 the cemetery management presumes that, if this purchase includes burial rights for future use by the purchaser(s), the type of burial site and/or burial rights selected indicates the desired disposition of the purchaser's(s') remains unless otherwise specified in this document.

Monthly payments to include interest at the rate of

a) Do not sign this contract before you read it,or if any spaces intended for the agreed terms, except as to unavailable information are blank. b) You are entitled to a copy of this contract at the time you sign it. c) You may at any time pay off the full unpaid balance due under this contract, and in so doing you may receive a partial rebate of the finance charge. d) The finance charge does not exceed twenty percent (20.00%) per annum, computed monthly. e) You may cancel this agreement if it is solicited in person, and you sign it at a place other than the offices of Associated Catholic Cemeteries shown above, which notice shall be posted no later than midnightof the third day (excluding Sundays and holidays) following your signing this agreement.

I understand that this agreement is my invoice and that payment is due as specified with no further notification from the cemetery. By signing this agreement I acknowledge my responsibility to perform the payment terms described herein. Time is of the essence. If any installment remains unpaid for a period of 60 days after it is due, a late charge of $25.00 (twenty-five dollars) per month will be added to the balance due hereunder so long as such delinquency exists. This agreement is not valid until accepted by Associated Catholic Cemeteries. Non-refundable Items Sold Pre-Need: Deposits to the Care Fund; items noted as "delivered/ installed" on this Agreement (shaded area indicates "delivered"); Private Mausoleums.
