REQUIRED DOCUMENTATION
Upload one (1) or more of the following for the Registrar's Office. The document will added to your file. Once the required documentation is received, your name will be updated.
Acceptable legal documents:
A signed court order for legal name change/adoption/divorce
OR A government issued passport showing the new legal name
OR Driver's license/ID AND social security card with new legal name
OR Marriage license AND social security card with new legal name
If students submit a name change mid-semester, students are responsible for notifying instructors after submission of this form to the Registrar.
To use a new/updated name on the diploma/certificate, the name change must be submitted prior to the last day of the final term. Any name changes after a diploma/certificate is printed, graduates must submit a Diploma Reorder for a one (1) time replacement under the new name. See that form for more details.
To only update the SSN/Tax ID Number: please only upload a copy of the Social Security/Tax ID Number card.
