By SUBMITTING THIS FORM, you agree to the following:
1) I give my permission to receive Vitamin B12 and or Kenalog injections.
2) I acknowledge that all information on this consultation form above my signature is accurate and correct to the best of my knowledge.
3) I accept full and complete responsibility for my own emotional and/or physical wellbeing both during and after this treatment.
4) I understand that the therapist does not diagnose illnesses or injuries.
5) I have clearance from my Doctor where necessary to receive the treatment I am booking for.
6) I fully understand the risks associated with B12 and/or kenalog injections.
I, therefore, release Emit elegance C.K and the individual therapist from all liability concerning these injuries/symptoms that may occur during/after the treatment session as I am aware of the risks.
7) I understand the importance of informing my therapist of all medical
conditions and medications I am taking, and to let the therapist know about any changes to these at any ongoing appointments. I understand that there may be additional risks based on my physical condition.
8) I understand that during the session I or the therapist may terminate the session at any
time.
9)Photos/Videos of your treatments may be taken to aid in record keeping, and to be used with your permission on social media to help advertise the services available
10) I am aware that our appointments are subject to late cancellation due to guidlines in place with CV19 regulations.
11) I have been given a chance to ask questions about the session
and my questions have been answered.
💫💫 Booking/Termination fee policy!
1) I am aware I will need to pay a 50% Booking/Termination fee within 12hrs of agreeing to the treatment date and time to secure the appointment slot or risk losing my slot.
2) I am aware I have the option to pay the Booking/Termination fee via bank transfer, PayPal or in cash directly to Emit elegance C.K.
3) I understand the Booking/Termination fee will be deducted off the total at appointment and that If I need to cancel or reschedule I must do so at least 48hrs before the agreed appointment time to be able to be refunded or have my Booking/Termination fee transferred over to my rescheduled appointment slot.
4) I understand that if I cancel after the 48hrs notice period/do not turn up to my appointment/are 15mins late I forfeit the Booking/Termination fee to Emit elegance C.K.
5) I understand that If in the event Emit elegance C.K has to cancel my appointment due to an event outside of their control e.g an emergency. I will be given the option to have the Booking/Termination fee transferred across to another slot or it will be refunded to me if I so wish.

Contracts for Services: Remedies.
Key notes:
A)"Reasonable care and skill” focuses on the way a service has been carried out, rather than the end result of the service.
B)"Services will be performed within a reasonable time" focuses on having services completed by the end of end of that day.
C)"Repeat performance" means Part of or all of service being done again as is necessary.
D)“Reduction in price" of an "appropriate amount" will normally mean that the price is reduced by the difference in value between the service the consumer paid for and the value of the services product costs and employees living wage.
1) I understand the first step I as the client must take is to request the Trader Emit elegance to fix (repeat performance) rather than demand a refund.
A) I understand that If the trader Emit.elegance CK breaches the term that the "services are to be performed with reasonable care and skill or in accordance with information provided by the trader"
B) I have a right to a repeat performance or a price reduction in certain circumstances. (see below).
C) I am aware that If the trader Emit.elegance breaches the term that the "services will be
performed within a reasonable time"
I have a right to a price reduction.
Right to repeat performance
2) I am aware I have the right to first require the trader Emit.elegance to perform the service again or repair, as is necessary to meet the terms of the contract.
a) I understand If I exercise this right, that the trader Emit.elegance must repeat the service within a reasonable time and without significant inconvenience to me and bear the costs of repeat performance.
b) I understand I cannot exercise the "Right to repeat performance" if it is impossible for the trader to complete the service in line with the contract.
Right to a price reduction.
1) I understand I can only exercise this right if
repeat performance of the service is IMPOSSIBLE or having received a request for repeat performance, the trader Emit.elegance has failed to carry it out without undue delay and without significant inconvenience to me.
a) I am aware that If I exercise the right to a "price reduction" when a "repeat performance" is IMPOSSIBLE the trader Emit.elegance must reduce the price by ‘an appropriate amount’.
b) I understand that In the event the price has already been paid I may be eligible for a PARTIAL REFUND.
C) I understand that once I have exercised this right, the trader Emit.elegance C.K must give me a PARTIAL REFUND within 14 days from the date that the trader Emit.elegance C.K agrees that
I am entitled to a price reduction. Unless otherwise agreed, the trader Emit.elegance C.K must pay the refund using the same method that you used to pay for the services. The trader cannot charge a fee for
making the refund..
D) I understand that if in the event I go to another "Trader" to have my service treatment "fixed,redone,removed" I can not request a Refund.
E) I understand that if the trader Emit.elegance has provided the service that I the client has asked for but do not like, the trader Emit.elegance has no obligation to provide a refund or partial refund.
F) I understand that if I develop an allergic reaction to any of the products used by Emit.elegance it is not the traders Emit.elegances fault and I can not request a refund.
COMPLAINTS PROCEDURE
In the unlikely event that you are unhappy please make Emit.elegance aware whilst you are in salon.
A) If you are unable to do so, please send a detailed email with timestamped pictures to c.k_emit_elegance@outlook.com as soon as possible.
B) After 7 days we may not be able to investigate/repair any problems.
