Cancellation/Rescheduling:

Working as a personal/independent hairstylist restricts appointment availability significantly compared to a traditional salon. B for Beauty reserves the right to charge 100% cost of an appointment changed without 48-hour notice. Canceling or rescheduling without ample notice takes away appointment availability for other guests, as last-minute openings often can not be filled. Illness and other emergencies do happen, therefore under special circumstances, 50% of the fee may be applied to a guest's following appointment.

By signing this form, I agree to these policies and authorize B for Beauty to charge my card on file for appointment changes made within 48 hours of my scheduled appointment time.

Pricing/Service Guarantee:

The pricing listed in the service menu is the starting costs for the services described. Each client's current hair and desired outcome are different and require different levels of product usage and time spent.

By signing this form, I understand that knowing the exact cost of a service is not typically possible until the service is completed and the pricing quote I have received prior to the service is subject to change upon completion. I agree to pay, in total, the final cost of any completed services I receive from B for Beauty.

Your stratification is of the utmost importance. Please understand that hair color services change with every wash and any adjustments or small corrections looking to be made need to be made within a small window of time.

By signing this form, I understand that I must notify B for Beauty of any desired changes within 72 hours of the completed service. Any major changes to a completed service will not be honored under a guarantee and I understand that I will be responsible for the cost of the service completed to change the desired look. I understand that a full blow-dry/style after a color adjustment service is an additional charge and agree to pay this charge in full.

Additional Guest/Children:

While B for Beauty takes pride in offering a personalized one-on-one experience catered specifically for each individual guest, ONE additional guest may join during a client session. Please do not expect additional guests to receive any services unless discussed during booking, including consultations. The scheduled time is devoted solely to the booked client and should be respected as so. The salon environment consists of chemicals and sharp objects that are not safe for children. Respectfully, it is asked that no children accompany a client or guest during a visit.

By signing this form I understand that no more than one additional guest and no children may accompany me to any hair service appointment at B for Beauty.

Chemical Services:

Chemical services can have varying results based on each individual's hair. It is crucial that any and all other processes that have been used on a client's hair are shared so that an informed decision can be made.
*** Please be aware that hair grows, on average, ½ inch per month. With an approximate 6-inch per year growth rate, chemical services performed even three or more years ago can still affect the outcome of your current service. 

Many factors play a part in the way A client's hair will react to a chemical service. In rare cases, it is unpredictable and may cause the service to turn out in a way that was not what was discussed or intended. If any of the following apply, please be hyper-aware of this possibility. It is absolutely vital that we discuss each one that applies, please notate any of the following concerns in the text box at the end of this form.

• The type of water used at home (well water, hard water, etc.)
• Medications 
• Previous chemical services (ANY coloring, keratin, etc.)
• Hormonal changes (changes in birth control, pregnancy, menopause, etc.)
• The products that are used for at-home care, or lack of products used

When visiting to correct hair color performed at home or by another salon, the chance of unexpected results is heightened. B for Beauty's first priority is always the integrity of the hair. When not privy to the full process and products used previously, it cannot be guaranteed that the service outcome will be exact to the desired result. However, the service will be given its best effort to begin the process of achieving the ultimate end goal, which may take more than one appointment. 

Clothing and jewelry may be exposed to chemicals during service, despite best efforts to protect them. B for Beauty is not responsible for any damage or changes to any clothing, jewelry, etc. present during salon visits.

I have read the above information and have/will make B for Beauty aware of any and all the processes I have used on my hair in recent years to assist in the assessment of my hair's current status and the process needed to achieve the desired results. I realize that this is very important information and that any information I withhold regarding my previous processes will increase the damage and the potential for unpredictable chemical reactions. I understand that chemical processes may cause some damage to the integrity of my hair. I understand that the products recommended to improve the health of my hair as well as maintain the results after the service are vital to the longevity and quality of my result. I am aware that B for Beauty has been trained in the service technique and that they will do their absolute best to create the best results, therefore I will not hold B for Beauty liable if the process has unexpected or undesired results. I understand that additional conditioning treatments may be recommended and/or necessary for my hair maintenance and that permanent damage to my hair is possible due to the chemical application.

I release and hold B for Beauty harmless against any and all liability, damage, and/or expenses arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) that I might incur as a result of the chemical altercation through permanent chemical processing of my hair.
