Please only fill this out if you already have a tattoo from me that you would like to be touched up.
It's totally normal for some tattoos to need touch ups! When I worked at Black Kat Ink, the touch up policy was free touch ups for up to 6 months after the tattoo. At my studio in Bountiful my policy going forward will be free touch ups for up to one year.

It has been over a year, the touch up is no longer free and will be at my normal rate.
