TWO PARENT SIGNATURES REQUIRED
This form must be completed by the student and his/her parents and submitted to the Georgia High School Fencing League, Inc. before a student is allowed to participate in the Georgia High School Fencing League or any team that competes in the Georgia High School Fencing League.
Parent’s or Guardian’s Permission and Waiver of Liability
We hereby give our consent for the below named student to participate in the Georgia High School FencingLeague. We hereby acknowledge and agree that participation in the Georgia High School Fencing League mayexpose the student to risks of property damage and bodily or personal injury, including injury that may be fatal.We understand that these risks may include, but are not limited to, sickness, broken bones, sprains, concussions,injury from athletic equipment and, heart attack. We further understand that the student may be exposed to otherrisks, which may not be foreseeable. We hereby assume any and all such risks.

We hereby give our consent for the below named student's likeness to be used in GHSFL publicity materials, including, but not limited to social media posts

We hereby agree to waive all liability of the Georgia High School Fencing League, its member clubs, officers,employees, volunteer(s), coaching staff, parent(s), sponsor(s) for any accident, injury, illness or other mishap thatmight befall the below named student while participating in any sponsored event including practice, tournamentsor fund raising activity.
Authorization for Emergency Care
We hereby grant permission to the coach and/or faculty sponsor, to any medical or surgical consultant deemedadvisable, and any hospital to render the below named student any medical and surgical treatment that they deemnecessary while we are not in attendance. We understand all possible effort will be made to inform us in case ofsuch treatment.
Covenant Not to Sue
We further covenant and agree that we will not sue either on our behalf or on behalf of the below named studentthe Georgia High School Fencing League, its member clubs, officers, employees, volunteer(s), coaching staff,parent(s), sponsor(s) for any damages resulting from or in any way connected with the student’s participation inthe Georgia High School Fencing League.
Applicable Law
I/we agree that this Release, Waiver of Liability and Covenant Not to Sue is to be construed under the laws of theState of Georgia and that if any portion is held invalid or unenforceable, the remainder shall, notwithstanding,continue in full legal force and effect.
Parent Understanding
We understand that participating in the Georgia High School Fencing League can be an extremely valuableexperience for students. The Georgia High School Fencing League makes every attempt to provide a safepractice and tournament environment. The Georgia High School Fencing League strictly enforces proper weaponhandling, wearing of protective gear and fencing etiquette. However, Georgia High School Fencing Leagueparticipants are exposed to moving objects, physical contact, stationary objects, various playing surfaces andother items that may cause injury and/or death. This communication is being written so you and the below named student can understand that there are potential dangers involved in participation in the Georgia High SchoolFencing League. All information given below is accurate.
