Informed Consent Form

I have read and understand the information sheet for the above study and have had the opportunity to ask questions.I agree to take part in the study as set out within the information sheet and understand that it has received ethical approval in line with the Research Ethics Policy and Procedures of Leeds Beckett University.I understand that my participation, data, and consent can be withdrawn without question from the study at any time up to the 30th of November 2023.I consent to interviews and my participation within the project being photographed, video and audio recorded.I understand that photographs, video and audio recordings of myself may be featured in the film ‘Civic Skateboarding’ and that I may not be able to inspect or approve the finished film prior to its release.I consent to any photographs, video and audio recordings of myself being used for promotional purposes by Harry Meadley, Leeds Beckett University, as well as the study’s partners and funders as stated in the information sheet.I understand that materials recorded in this study may be used within a future PhD thesis or publications by Harry Meadley, as well as for future funding applications, presentations and exhibitions.I understand that the act of skateboarding is at my own risk and the Civic Skateboarding study is not responsible for any accidents or injuries obtained while skating in a public space.
