  • THIS AGREEMENT, entered into this 1st day of September 2022 between the parties to the PROGRAM REQUIREMENTS FOR THE REGISTERED YOUTH/PRE-APPRENTICESHIP PROGRAM IN THE CONSTRUCTION INDUSTRY represented by the YOUTH/PRE-APPRENTICESHIP COMMITTEE, herein referred to as the Academy of Construction Technologies, Inc. (ACT), and {youthpreapprenticeFull} hereinafter referred to as the YOUTH/PRE-APPRENTICE.

  • WITNESSETH THAT:

    The Youth/Pre-Apprenticeship Program Committee agrees to be responsible for the selection, placement, and training of said Pre-Apprentice in the CONSTRUCTION INDUSTRY as work is available, and in consideration said Pre-Apprentice agrees diligently and faithfully to perform the work of said specific construction trade in accordance with the PROGRAM REQUIREMENTS FOR THE YOUTH/PRE-APPRENTICESHIP PROGRAM IN THE CONSTRUCTION INDUSTRY. The ACT Youth/Pre-Apprenticeship PROGRAM REQUIRMENTS referred to herein are hereby incorporated in and made part of this agreement.

    This agreement may be terminated by mutual consent of signatory parties.

  • PARTICIPATING HIGH SCHOOL YOUTH/PRE-APPRENTICESHIP AGREEMENT between parent, student, contactor and high school.

    PROGRAM PURPOSE: To provide students with academic and construction trade skills necessary to successfully complete a youth/pre-apprenticeship trade of their choosing.

    Parent/Guardian Agreement

    I, the parent or guardian, agree that this youth/pre-apprenticeship program in the construction trades is in the best interest of my son/daughter to pursue. Furthermore, I have read the below goals and agree with their intent.

    Goals:

    1. Meet state requirements for high school graduation.
    2. Develop job entry skills.
    3. Learn trade skills in a selected specific construction trade.
    4. Successfully enter the construction trade in the area he/she has demonstrated interest and aptitude.

    I acknowledge a copy of the PROGRAM REQUIREMENTS for the youth/pre-apprenticeship program in the construction trades is posted on the ACT website at www.ACTCAREERS.com and have read the provisions therein. I further understand that all questions and contacts pertaining to the program between the school and myself or between the contractor/employer and myself are to be made through the classroom instructor or ACT's executive director. I further understand that my son/daughter must work 160 hours successfully for out- of-class; on-the-job work experience credit. Finally, I understand that the PROGRAM REQUIREMENTS referred to herein are hereby incorporated in and made a part of this agreement.

  • STUDENT AGREEMENT

    I agree that the pursuit of the above Youth/Pre-apprenticeship program goals meets my interest now and for the foreseeable future. I agree to cooperate with my school's classroom instructor, ACT's executive director, contractor/employer, and parent/guardian in fulfilling my responsibilities to meet these conditions stated in my parent's agreement above, and the PROGRAM REQUIREMENTS for the youth/pre-apprenticeship program in the construction trades were explained to me by the classroom instructor & ACT's executive director.I furthermore acknowledge a copy of the PROGRAM REQUIREMENTS for the youth/pre-apprenticeship program in the construction trades is available for my review in my classroom or posted on the ACT website at www.ACTCAREERS.com. I understand that my performance in the youth/pre-apprenticeship program classroom, laboratory, and on-the-job are all a part of the evaluation process for grading and continuance or termination from the program.

  • The ACT program periodically takes photographs and/or video tape productions of the ACT students during participation in their construction-related classes, student/employer job fairs, summer on-the-job training, and end-of-the-year graduation banquets.

    The undersigned hereby assigns the Academy of Construction Technologies, Inc., its successors, and assign for valued received, any and all rights in such recorded image or audio recordings for use to which such image or recording may be put including, but not limited to, instruction, promotion advertising, Website/Internet and public relations with our without reference to my name of the organization. The undersign also release the Academy of Construction Technologies, Inc. from any and all claims which would or might arise out of such image recordings for invasion of privacy, false rights, misappropriations of image or likeness, or any other cause of action arising out of the taking, recording, or use of such image(s) or recording(s

    Student Learner Exemption Agreement

    (This form is offered to all registered ACT students and acknowledges the student's participation in a recognized youth vocational training program. Through this program, you can perform jobs that they would typically be restricted from performing)

    The following are occupations for which the exemption applies: 

    X - On a scaffolding, roof, superstructure, residential or non-residential building construction, or ladder above 6 feet.

    X - In the operation of power-driven woodworking machine.

    X - In the operation of power-driven metal forming, punching, or shearing machines.

    X - Excavation operations.

    X - Working on electric apparatus or wiring.

    X - Operating or assisting to operate, including starting, stopping, connecting or disconnecting, feeding or any other activity involving physical contact associated with operating, tractors over 20 PTO horsepower, any trencher or earthmoving equipment, forklift, or any harvesting, planting, or plowing machinery, or any moving machinery.

    In accordance with Section 450.061(2), the undersigned attest to the following:

    1. The student learner is enrolled in a youth vocational training program under a recognized state or local educational authority
    2. The work of the student learner in the occupation declared particularly hazardous is incidental to the training received.
    3. That the work performed shall be intermittent and for short periods of time and under the direction and close supervision of a qualified and experienced person.
    4. That safety instruction shall be given by the school and correlated by the employer with the on-the-job training.
    5. That the student has a schedule of organized and progressive work processes to perform on the job.

    PRE-APPRENTICESHIP AGREEMENT: Between the Pre-apprentice and the Pre-apprenticeship Program SponsorTHIS AGREEMENT, entered into this 1st day of September 2022 between the parties to Academy of Construction Technologies represented as the Pre-apprenticeship Sponsor and PRE-APPRENTICE, and (if a minor) Parent or Guardian.

    WITNESSETH THAT: The Program Sponsor agrees to be responsible for the selection, placement and training of said pre-apprentice, as work is available, and in consideration said pre-apprentice agrees diligently and faithfully to perform the work of said trade during the period of pre-apprenticeship, in accordance with the registered standards of the Program Sponsor. The pre-apprenticeship standards referred to herein are hereby incorporated in and made a part of this agreement. This agreement may be terminated by mutual consent of the signatory parties, only upon proper notification to the Registration Agency.

    Warning: This Pre-apprenticeship Agreement does not constitute an Apprentice Certification under Title 29, CFR, Part 5 for the employment of the Pre-apprentice on Federally financed or assisted construction projects. No Certifications can be obtained from the Registration Agency's Servicing Representative.

    By signing below you acknowledge your full knowledge of the provisions and my rights thereunder, do hereby expressly waive my rights under 20 USCA S1232g(b) which provides that a student's permission (or the permission of his/her guardian, if the student is under 18 years of age) is necessary before an educational agency or institution may disclose the student's education records to any source outside the school system. Permission to disclose my records (or my child's records) is specifically restricted to the disclosure of grades and attendance records to the Registration Agency for the purpose of evaluating my progress as a pre-apprentice and further administering of the Florida Pre-apprenticeship Program provided for under Chapter 446, Florida Statutes.
