Are you aware that this form is to be completed by a parent / carer applying for their child to receive the Sacrament of *CONFIRMATION* and that the sacrament of Confirmation is for children in Year 8 of Secondary School and above? *

- Tuesday 4th October 2022 - 7:30pm - Corpus Christi Church Hall
- Tuesday 11th October 2022 - 7:30pm - Corpus Christi Church Hall
- Tuesday 18th October 2022 - 7:30pm - Corpus Christi Church Hall
Through completion of this form you agree to attend all of the sessions listed above.

Failure to do so will mean you are unable to apply for your child to receive the Sacrament.
