Corpus Christi Sports Club Badminton Waiver Form

All players are required to sign a liability waiver before accepted as a visitor or as try-outs. For players under 18, their parent(s) are required to sign the waiver. Those who choose not to sign it will not be accepted as CCSC visitors or as try-outs.

Tryouts/Visitor/Drop-in Rules Revision:

1. Unless granted special permission, walk-in visitors are not permitted.

2. Only first-time tryouts may participate for free.

3. All visitors/drop-in must sign a waiver, obtain approval from a director, and pay a $15 advance fee to play for a single day.

4. Visitor/drop-in and tryout sessions are held exclusively on Thursdays at Asbury Church. No exceptions.

CC SPORTS CLUB AGREEMENT AND RELEASE

Agreement

The Corpus Christi Sports Club (“CC Sports Club”) has organized badminton and volleyball activities for its members and leased space and time in facilities where such activities occur in reliance on the following agreement and release to be executed and delivered by each Participant prior to participating in any CC Sports Club activity.

Participant acknowledges and agrees that his/her participation in the CC Sports Club is voluntary and Participant may elect not to participate in the CC Sports Club at any time. Participant further acknowledges and agrees that the CC Sports Club may elect to terminate his/her participation in the CC Sports Club at any time for any reason in the discretion of the CC Sports Club.
Participant acknowledges and agrees that it is Participant’s responsibility to obey any and all rules and regulations of the CC Sports Club and of the facility where such activities occur.
Participant acknowledges and agrees that the safety of Participant is Participant’s responsibility and Participant hereby assumes any and all risk associated with participation in the CC Sports Club.
Participant acknowledges and agrees that Participant is responsible for any damage to the facility or its equipment that is caused by Participant.

Release From Liability

PARTICIPANT HEREBY RELEASES THE CC SPORTS CLUB AND EACH AND EVERY ONE OF ITS ORGANIZERS AND MEMBERS (THE “CC SPORTS CLUB PARTIES”) AND AGREES TO HOLD THE CC SPORTS CLUB PARTIES HARMLESS FROM AND AGAINST ANY CLAIMS OR CAUSES OF ACTION OF ANY NATURE WHATSOEVER, INCLUDING, BUT NOT LIMITED TO, ANY CLAIMS FOR PERSONAL INJURY, THAT RESULT FROM OR IN ANY WAY ARISE IN CONNECTION WITH ANY PARTICIPATION IN THE CC SPORTS CLUB. PARTICIPANT GRANTS THIS RELEASE OF HIS/HER OWN FREE WILL AND DOES SO IN EXCHANGE FOR BEING ALLOWED TO PARTICIPATE IN THE CC SPORTS CLUB. THE CC SPORTS CLUB PARTIES ARE ALLOWING PARTICIPATION IN THE CC SPORTS CLUB BY PARTICIPANT ONLY IN RELIANCE ON THIS RELEASE AND WOULD NOT ALLOW SUCH PARTICIPATION IN THE ABSENCE OF THIS RELEASE.
