General Information
This practice utilises therapies grounded in the principles of evidence-based treatments. Where clinically appropriate we may also include animals such as horses, dogs and wildlife in an outdoor setting. This presents a unique situation carrying with it additional risks not found in other clinical settings. When working outdoors, clients may be observed by others outside of the therapeutic space, this will be monitored and consultation, planning and management between client and therapist will occur as required.
Participants must wear suitable outdoor clothing, and sturdy shoes. We are in a farm environment and instructions will be provided where necessary regarding safe practices. Participants should note electric fences are in use on this property and should not be touched.
Provider’s Responsibilities
Wedgetail Therapy promises to:
- listen to you
- be reliable, honest and respectful
- work in partnership with you and other service providers
- work with you on the changes that are important to you
- support you to make decisions with confidence
- share current information and knowledge with you
- be clear about what we can do
- listen to your feedback and resolve problems quickly
- once agreed, provide supports that meet your needs at the time and place you prefer
- give you 48 hours’ notice (where possible) if we need to change a scheduled appointment or full payment is to be made
- review the provision of supports with you at least 6 monthly
- keep clear and accurate records on services provided to you
- to provide 14 days’ notice of price increases
- issue regular invoices and statements of the supports delivered as per the Terms of Business for Registered Providers.
Participant or Participants Representative (parent/guardian) Responsibilities:
- work with Wedgetail Therapy to ensure that the services and supports delivered meet our needs
- treat Wedgetail Therapy staff with courtesy and respect
- refrain from bringing dogs or other animals onto the property
- talk to Wedgetail Therapy if I have any concerns about the services or supports being provided
- give at least 48 hours’ notice when I need to cancel an appointment; and if notice is not provided Wedgetail Therapy cancellation policy will apply, that is full amount will be invoiced.
- let Wedgetail Therapy know immediately if my NDIS plan is suspended or replaced, or I stop being a participant in the NDIS.
To enable Wedgetail Therapy to meet its obligations under the program, I understand that Wedgetail Therapy may need information about any other services client receives. I agree to provide such information to Wedgetail Therapy on request and to inform Wedgetail Therapy of any changes to the services being provided.
I give permission to the therapeutic team at Wedgetail to provide feedback to primary contact or Support Coordinator on any matter noted in the therapeutic encounter that gives rise to concern or constitutes important information in the overall delivery of services for that client. This is in addition to mandatory reporting requirements in place. Primary contact for this purpose will be provided on the Contact form in the Welcome pack.
Feedback and Complaints
I understand I have the right to provide feedback or make a complaint about the service provided by Wedgetail Therapy and to have that complaint investigated promptly and fairly. To provide feedback or make a complaint please refer to ‘Feedback and Complaints’ guidelines provided to you at intake or phone Wedgetail Therapy Customer Service on 0417 312 602.
If the matter cannot be resolved, or the participant is not happy with the response to their complaint they are able to make a complaint about the provider to the NDIS Quality and Safeguards Commission by:
The NDIS Commission can take complaints from anyone about:
- NDIS services or supports that were not provided in a safe and respectful way
- NDIS services and supports that were not delivered to an appropriate standard
- how an NDIS provider has managed a complaint about services or supports provided to an NDIS participant
Confidentiality
Personal information gathered as part of this service will remain confidential except when:
- it is subpoenaed by a court; or
- failure to disclose the information would place you or another person at serious risk to life, health or safety; or
- your prior approval has been obtained to:
a) provide a written report to another professional or agency. e.g., a GP or a lawyer; or
b) discuss the material with another person, eg. a parent, employer or health provider; or
c) disclose the information in another way; or
-
you would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected; or
-
disclosure is otherwise required or authorised by law.
Photography
As a standard practice, photographs are taking during sessions and digital copies of the images are provided to the client/parent as part of the session feedback process. These photos will not be provided to anyone else and are stored confidentially along with all other forms of documentation. Please speak to your therapist if you do not wish to have photographs taken. Where appropriate, and with client consent, some photos may be used on our social media platforms.
