• Justification for Equipment and Supplies

  • Shay and Dani's Rate:
    Option A: Just signature: $110

    • The therapist will sign a pre-filled JFE.  They will quickly look through before signing but we cannot provide resigning or refund if the JFE is declined.


    Option B: Writing or editing: $220 (includes the signature)

    • The therapist will fill, edit, sign the JFE.  Family must still provide preceise name, link, and price to the qualifying items you wish to perchase below. If the JFE is declined, the therapist will edit and resign for free.
