In accordance with the membership agreement, 30 (thirty) days notice is required to cancel your membership. Your membership will remain active during the 30-day period and you may use the club until the cancellation date noted below. You must be in good standing: all outstanding amounts, dues and cancellation fees must be paid prior to cancellation. A designated Anytime Fitness Boothwyn staff signature is required on this form before cancellation requests are approved and there is no back-dating. Month-to- month and agreements beyond their initial term are not subject to an additional cancellation fee beyond the 30-day notice. Members terminating their agreement before the term ends will be subject to a fee equal to 50% of the amount remaining on their agreement or $150, whichever is greater. Members relocating to an area where there is no Anytime Fitness club within 10 miles of their new address will be subject to a flat $50 or $100 early termination fee, depending on the months remaining on the agreement, plus any payments due during the 30-day notice period. Adequate proof of new address is required-an official copy of your lease agreement or mortgage with your name on it, or a utility bill with the new address and your name. Anything on this form that may contradict the terms of your agreement may be considered null and void.
