Based on knowledge gained from the Phase I activities of the PLU, submit a Plan of Action to create a high-performance learning culture in your school or district.
The plan must have a target goal that is SMART (specific, measurable, attainable, results-oriented, and time bound) for the school or district.
The Plan of Action will include:
- Target Goal
- Action steps
- To whom each step should be directed
- How each step will be implemented
- Time Frame for implementation of each step
Use the following template for your plan. You may add as many rows as needed for your time frame. Make sure all information is visible in all cells before submitting.

When complete, you will need to download or save your Plan of Action as either a PDF or Excel file. You may then upload the file into this form.
