Form must be filled out to completion for your submission to be considered. Submission of this form does not guarantee an appointment.

A non-refundable deposit of $100 is required at the time of scheduling. An invoice will be sent to your email via Square payment system.

Note that I do not tattoo all styles or subject matter. Be sure to read the FAQ section for my website to see if your design is a good fit for my style of work.

Be specific, for example: side of left thigh, outer left arm, etc. PLEASE NOTE I am not accepting chest or torso placements at this time (with the exception of shoulder/ upper back placements.)

All designs are custom- I will not copy an image directly. References may be used to inspire the design, or explain design elements you'd like included. Feel free to include a loose sketch of your idea- it doesn't have to be good but it can be very helpful to convey your idea. Please do not send AI generated references.

Have a friend help take the photo for best results. Send a well lit, straight on photo of the area. PLEASE rotate the photo if taken in self-portrait mode. For coverups and reworks, a picture of the original tattoo MUST be included.

Minimum for custom designs is $200. I will discuss with you if I cannot accommodate within your budget for the described design or requested size.

I will do my best to accommodate travel dates but cannot guarantee specific availability. Please note that I may be scheduled 3-6 months in advance.

You will need to check with a doctor if you are taking blood-thinners, accutane, antibiotics, immonusuppressants, or are diabetic.
