Positive Vibes Only Bubble Soccer Waiver

In consideration of being allowed to participate in any way in Positive Vibes Only, Corp Bubble Soccer and related events and activities, the undersigned acknowledges and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. Specifically, I understand the following activities may result in the following corresponding injuries:
   1. Positive Vibes Only, Corp bubble soccer - Competing in American soccer, encapsulated inside a giant bubble ball. Players are trying to score goals, but as part of the game, are bumping into each other with impact. Accordingly, possible injuries include sprained ankle, wrist, concussion, bruising, cuts, nausea, dizziness, fatigue, and broken bones,
3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and reasonably foreseeable, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
4. I certify that I am in sound health for vigorous physical activity, am not pregnant, and have valid medical or health insurance coverage in effect at this time, and there is no reason why I cannot participate in this athletic program; and
5. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of a program leader immediately; and,
6. I RELEASE AND HOLD HARMLESS Positive Vibes Only, Corp, Ramone Griffith, program leaders, assistant volunteers, or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. Such release applies to whether Positive Vibes Only, Corp’s services are rendered offsite or on Positive Vibes Only, Corp’s premises ; and,
7. In the event that Positive Vibes Only, Corp is found liable for participant’s injury, participant’s damages shall be limited to compensatory damages.
8. I consent to Positive Vibes Only, Corp and their representatives taking photographs or digital recordings of me as a participant during this event and use these in any and all media for training or promotional purposes. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration.
9. I agree that if I damage any property of Positive Vibes Only, Corp, including but not limited to - Camera's, Camcorders, Bubble Balls, Nets, and pumps, due to negligence, sharp objects on my person, or running into a sharp object that can puncture a bubble ball, I can be held responsible and charged for the cost of the damaged property. Intentionally damaging the equipment, not following the rules of the game, safety rules, or Positive Vibes Only, Corp Staff members, will result in immediate charge of the equipment damaged.
10. I agree and understand that I must listen to all Positive Vibes Only, Corp employees at all times for my safety. I will not be rude or go against any rules of the employee’s, facility, or other participants. Positive Vibes Only, Corp reserves the right to ask anyone that is rude, or negligent to leave the premises, or discontinue their participation in the activities.

FOR PARENTS OF PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and certify my child(ren) in appropriate physical health as outlined in the above paragraph 3, has health insurance, and consent to usage of their photographs and likeness for promotional purposes.
