Permission Slip for Educational Field Trip 22-23

Dear Parent/Guardian:

During the 2022-2023 school year, students enrolled in the ALC seat-based program may be taking field trips to various places in the Duluth/Superior area. These trips will encourage community building amongst students and staff, as well as be included as enrichment activities for the classroom.

The Board of Education requires that we have written permission from each parent/guardian for their child to take these field trips. If you are willing to have your child take part in both the educational field trips and fun excursions, please sign this document for our school to keep on file.

This permission slip MUST be signed and returned for students to participate in field trips. Please return permission slips as soon as possible.

It is understood that the assigned person in charge will take reasonable precaution to guard against any accident or injury occurring to the students. It is understood that the student must abide by the directions given by the assigned person in charge at all times. The assigned person in charge is authorized to secure the services of a doctor or hospital should it be deemed necessary for the well being of the child.

Parent/Guardian will assume all responsibilities and expenses not covered by school insurance.
