Informed Consent and Acknowledgement I hereby give my approval for my (and/or my child’s) participation in any and all activities prepared by Peaceful Warriors during the selected event.
I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Peaceful Warriors, the venue, instructors, volunteers and representatives from any and all liability for injuries to myself and/or said child arising out of traveling to, participating in, or returning from selected sessions. In case of injury to said child, I hereby waive all claims against Peaceful Warriors, including all teachers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all activities, including yoga.
I understand photographs and/or video may be taken at the event to use in advertising and to document the event for our sponsors and donors. I will make it clear to the staff if I or my child may NOT be photographed.
