If your pet has had a seizure, and is a current patient of CVNI, please use this form to report the seizure. This information will be used to record the frequency of your pet’s seizures, and is not meant to be used to report an emergent situation.

When your pet needs immediate care, please take him/her to the nearest pet hospital for emergency treatment.
