  • PARENTAL CONSENT (signature required)

    I.  The undersigned does hereby give permission for our (my) child to attend and participate in activities sponsored by St. Catherine of Siena’s YOUTH MINISTRY Program. 
    II.  We (I) authorize an adult, in whose care the minor has been entrusted, to consent to any X-ray examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any licensed physician or dentist. The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medical and dental services rendered. 
    III.  We (I) hereby grant permission for non-prescription medication to be given, if deemed appropriate. 
    IV.  Should it be necessary for our (my) child to return home due to medical reasons, behavioral reasons, or otherwise the undersigned shall assume all transportation costs. 
    V.  The undersigned does also give permission for our (my) child to ride in any vehicle designated by the adult in whose care the minor has been entrusted while attending and participating in activities sponsored by St. Catherine of Siena’s YOUTH MINISTRY Program. 
    VI.  I give permission for my teen to be photographed during activities associated with St. Catherine of Siena’s YOUTH MINISTRY Program. I understand that said photos/videos may be used for future YOUTH MINISTRY publications within the St. Catherine of Siena Community and social media.
    VII.  I give permission for my child to be contacted via text, phone call, email, or groupme by the staff of St. Catherine of Siena. 

  • ASSUMPTION OF THE RISK RELATING TO COVID-19

    The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person to person through respiratory droplets when an infected individual coughs, sneezes or speaks. As a result, government agencies at all levels and federal, state and local health agencies recommend social distancing and have placed limits on the congregation of groups of individuals.

    St. Catherine of Siena Parish has put preventative measures in place to reduce the spread of COVID-19; however, the parish cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending activities on the campus of St. Catherine of Siena Parish could increase your risk and that of your children for contracting COVID-19.

    While St. Catherine of Siena Parish will make all reasonable efforts to lower the risk of COVID-19 exposure and spread at the parish, the parish is unable to provide any guarantee that children or their families will not be exposed to or infected by COVID-19.

    By agreeing to have your child(ren) serve at Mass, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you, your child(ren) and or other family members may be exposed to or infected by COVID-19.  It is expected that children and other family members will follow the preventative measures and guidelines implemented by St. Catherine of Siena Parish including not coming to the parish premises if demonstrating any signs or symptoms of COVID-19.
