KC Pet Project Release of Liability and Consent for Birthday Party

NOTICE: THIS IS A LEGALLY BINDING AGREEMENT. Read this document carefully and in entirety. By signing this agreement, you give up your right and the named minor(s)’s right to bring a court action to recover compensation or obtain any other remedy for any personal injury or property damage however caused arising out of the named minor(s)’s participation in KC Pet Project education programs, now or any time in the future.

I, as parent/guardian of
child(ren)* ren)”), hereby assume all risks and hazards incidental to the conduct of the activities at KC Pet Project. I understand that physical activity carries the risk of harm and injury. I understand that in handling and being in the presence of animals there does exist a risk of injury including physical harm caused by the animals. I represent that my child(ren) has/have received a tetanus vaccination and/or all vaccinations recommended by our physician. I understand that no insurance coverage for participants in these activities is provided by KC Pet Project. In consideration for my child(ren)’s participation in the KC Pet Project event, I agree that on behalf of myself, my child(ren), my heirs, family members, personal representatives and executors, we assume the risk of the activities and I release, discharge, indemnify, and hold harmless the KC Pet Project, its agents, employees, officers, and directors from any and all claims, causes of action, or demands, if any, in connection with the same, based on damages or injuries which may be incurred or sustained by me or said child(ren) in any way connected with said child(ren)’s participation in activities or presence at the KC Pet Project. I give the employees and/or agents of the KC Pet Project authority to seek emergency dental/medical and/or surgical transport and treatment for said child(ren). I understand that I and the named minor(s) will be solely responsible for any loss or damage, including personal injury, property damage, or death, the named minor(s) sustains while participating in education programs and that by signing this agreement I, on behalf of myself and the named minor(s), HEREBY RELEASE KC Pet Project of all liability for such loss, damage, or death. I further certify that the named minor(s) is in good health and has no conditions or impairments which would preclude his/her safe participation in education programs.

Please be aware that most of our enrichment treats used in house and made in the Youth Enrichment Program, and VIP birthday parties use peanut butter. Please let us know before your visit if anyone attending has a peanut allergy so that we can make other arrangements. KC Pet Project will not be responsible for any allergic reaction if we have not been made aware of allergies in the group before arrival.

Coronavirus / COVID-19 Warning & Disclaimer
Coronavirus, COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal and state authorities recommend social distancing as a mean to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in KC Pet Project programs or accessing KC Pet Project facilities could increase the risk of contracting COVID-19. KC Pet Project in no way warrants that COVID-19 infection will not occur through participation in KC Pet Project programs of accessing KC Pet Project facilities.

Photographic Image Release
I, the undersigned, hereby grant permission to KC Pet Project and or other participating parties to take and use photographic images of myself or my child(ren) during this event for the express purpose of publicity regarding the KC Pet Project programs, or otherwise at the discretion of the KC Pet Project.

Request and Permission to Participate

I hereby request and grant my permission for my child(ren) to participate in this event and all its associated activities, and agree to the releases and consents set forth above. The terms of this agreement are severable and each shall be effective if another part is invalid.
I HAVE CAREFULLY READ THIS LIABILITY WAIVER AND FULLY UNDERSTAND AND AGREE TO ITS CONTENTS. I AM AWARE THAT BY SIGNING THIS DOCUMENT, I AM GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING, WITHOUT LIMITATION, THE RIGHT TO SUE THE CHARLESTON ANIMAL SOCIETY, ITS EMPLOYEES, OFFICERS, DIRECTORS AND AGENTS, ON BEHALF OF MYSELF, MY CHILD(REN), MY HEIRS, FAMILY MEMBERS, PERSONAL REPRESENTATIVES AND EXECUTORS.
