TRANSPORTATION CONSENT AND LIABILITY RELEASE I hereby grant permission for my child(ren) named above to participate in any and all events sponsored by the parish that occur away from the parish site and that may or may not require transportation to and/or from said event. This means any event sponsored by St. Elizabeth Ann Seton Catholic Church and the Office of Youth and Young Adult Ministries of the Diocese of Lexington between July 1, 2022 and June 30. 2023.
These activities will take place under the quidance and direction of parish employees and/or volunteers from St. Elizabeth Ann Seton Catholic Church in Lexington KY. These activities include but are not limited to the following trips. All trips unless otherwise stated are from 6:30pm to 8:00pm and do not include transportation. All trips are for students in the 6th-12th grade. Friends are always welcome.

5/5-5/7 HS retreat (5pm Friday - 12pm Sunday at bluegrass christian camp Transportation by bus)
The mode of transportation to these events may include walking, public transportation, personal vehicles, and/or chartered bus. Transportation vehicles may be driven by designated adult parish employees and/or volunteers. As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above named child(ren)/ participants. And therefore agree on behalf of myself, my child named herein, and our heirs, successors, and assigna, to hold harmless and defend St. Elizabeth Ann Seton Catholic Church and Thomas Cross Missions in Lexington, Ky. its officers, directors, employees, and agents, and the Diocese of Lexington, its employees and agents, chaperones or representatives associated with the event from any claim arising from or in connection with my child attending the event or in connection with any injury (including death) or cost of medical treatment in connection therewith and agree to compensate the parish its officers, directors, and agents and chaperons or representative associated with the évent for reasonable attorney's fees and expenses which may incur in any action brought against them as result of such injury damage, unless such claim arises from the negligence of the parish/diocese.
