Request for Accommodations

 

The Department of Student Affairs at the Art Academy of Cincinnati provides services and/or accommodations for students with health needs and/or disabilities in an effort to facilitate equal access to education opportunities. To determine eligibility for services and/or accommodations, current and comprehensive documentation regarding the physical or mental condition and its impact on the student’s functioning is required from a licensed medical professional qualified to diagnose and treat the particular condition(s).

If you have an IEP (Individualized Education Plan) or 504 plan, you do not need to have a medical professional complete this form. Please attach your IEP to the end of the form OR email it to mmartin@artacademy.edu . Please keep in mind that the accommodations you receive may not be identical to the accommodations in your IEP or 504 plan. If you DO NOT have an IEP, a licensed medical professional must complete the information below.

Accommodation Requests for Fall must be submitted by August 15.

Accommodations Requests for Spring must be submitted by January 5.
