Community Needs Assessment
The Family Tree Program, administered by CCD Counseling since 1997, is the local version of the state-wide Family and Youth Success (FAYS) program.

The intent of this program is to promote strong families and resilient youth, whatever that might look like for each family. The Family Tree Program attempts to accomplish that through the provision of family counseling and skills-based groups for both caregivers and youth.

We are always trying to improve the ways we serve our communities. This survey will inform the next version of FAYS programming for our community. Thank you for helping us.
