    Register your team fundraiser idea for approval and setup
  • All Fundraisers are in coordination with the Lakewood Athletic Boosters and must be approved by the Athletic Director. 

    Each team can do 3 Fundraisers per School year (June 1 trough May 31).

    Team Spirit Packs, Fan Spirit Wear do not count as Fundraisers but please complete the form so we can get you set up!

  • Important Dates

    Provide the dates that this Fundraiser start to be promoted/accepting payments and when the fundraiser is over. Your Team Fund Deposit will be due 3 days from the end date.

    Check the Teams that are involved in this Fundraiser (splitting the profits if multiple).

  • Tax Deductible Donations

    Will a company or people make Tax Deductible Donations for this? If so, we may need to provide them with the Boosters 501c3 documents.

  • Describe the Fundraiser

    Include who/what/where and what kinds of online payments you need set up. What school facilties may be used, whether a Concession Stand needs to be operated, etc.
