Transform Church Marriage Application
This form should be used by a couple who is engaged and would like Transform Church to do their premarital SYMBIS program. PLEASE fill out this application together. SYMBIS stands for saving your marriage before it starts. SYMBIS+ is for married couples looking to improve their marriage. There are questions for the bride and groom to personally fill out regarding their reasons for marrying and their salvation testimony... Please expect this form to take at least 15 to 30 minutes to fill out. There is a premarital fee of $300 that covers the premarital sessions and material with the minister/facilitator, plus there is additional SYMBIS registration site fee $35 for the couple. For the Transform Church Minister performing the wedding ceremony, there is an honorarium starting at $350 plus extra time and travel expenses. The word of God defines marriage being a covenant between a man and a woman in a life long commitment.

After completing this form, send an email to Alex@TransformChurch.com requesting the Church Lyndhurst Foyer for your vows. There are limitations for the use of the foyer. We don't use the other rooms for weddings and one of our ministers must officiate the wedding.

Did you both completed this form together and read our Church Policy on marriage and accept the terms, including the fee for Transform Church premarital and the fee for a marriage official? If you would like to use the Church Foyer for your wedding, please provide the following information: Reason for choosing the foyer: Tell us why you would like to use the church foyer for your wedding. (For example: sentimental value, love the look of the space, convenience, desire to introduce your family to Transform Church, need to marry soon, financial hardship, or other reasons.) How you heard about it: Please share how you found out that the church foyer may be available for weddings. Guest count: The church foyer can accommodate a maximum of 26 people. Please indicate the number of guests you expect. Date and time: Foyer weddings can only take place on Fridays or Saturdays between 11:00 AM and 1:00 PM. Please list your preferred date and any alternate dates. Setup and cleanup: Your party is responsible for setting up chairs and resetting the space to its original condition after the ceremony. Approval: You will be notified once your request for the selected date has been reviewed and approved (or not approved).
