  • The following is our policy for all pet(s) boarding or grooming at our facility:

    Vaccination Requirements

    Dogs must have been vaccinated within the last 12 months against rabies, DHPPC, Bordetella, Canine Influenza (H3N8, H3N2).

    Cats must have been vaccinated within the last 12 months against Rabies, FVRCP.

  • Hospital Policies

    • It is the owner’s responsibility to provide us with proof of the above information. Any pet not current on vaccinations will be brought up to date upon admittance. An estimate will be provided for any vaccinations due. This is for your pet’s protection and the protection of other animals in our facility.
    • If fleas or ticks are present upon admittance, the pet will be treated immediately and charged accordingly.
    • The hospital and staff are not responsible for the return of personal items. All leashes and collars will be given back to the owner at drop-off.
    • All hospitalized, boarding, and grooming pets will be admitted and released to their owner during routine office hours, NO EXCEPTIONS! Pets not picked up by 6 pm will need to stay the night in our boarding facility and the owner will be responsible for boarding charges.
    • By initialing below, you give permission for your pet to be treated by the veterinarian in the event of an emergency. Every attempt will be made to contact the emergency contact listed on the account. But if the emergency contact cannot be reached, you understand that your pet will be treated, and you will be responsible for all costs incurred.
  • Grooming Policies

    • Grooming instructions will be noted on the patient’s account and will be used for all future grooms. If any changes need to be made, please notify the front staff so they can notate changes.
    • The groomer does reserve the right to deny a pet for grooming based off the temperament of the pet. Some pets may be fearful for grooming and may require some level of sedation. If this is need for your pet, a member of out medical staff will contact you to discuss options.
    • I understand that if my pet is severely matted that they will need to be shaved. BPAH staff will notify you prior to grooming.
    • Once your pet’s grooming appointment is complete you will receive a notification via text message that your pet is ready for pick up.

  • Boarding Policies

    • We require all food to be pre-portioned in individual containers prior to drop off. Each container must be labeled with the pet's first and last name. All canned food must be labeled. If the pet is free-fed, please bring food in a large Ziploc bag. Large food containers will not be excepted. Do not add medications or supplements to the bag of food. You may bring treats but please place these in a separate bag. We cannot guarantee that treats will be given during stay. Please send a few extra days of food in the event you cannot pick up your pet at the predetermined time.
      •Boarding can be a stressful time for some pets. This may cause them to not want to eat. In the event your pet is not eating well during their stay, we will use gastrointestinal-friendly canned food to entice them to eat at an additional cost to you. If your pet has a food allergy you will need to notify the staff to not feed them any additional food. Due to increased stress and decreased activity, some pets may not eat all the food that is brought. In that event, we will return any unused food.
    • Medications and supplements will need to be brought in original prescription vials or containers. Any medications not brought in original containers will not be accepted. All medications will be verified by the medical staff upon arrival.
    • A fee of $2.00 per dosing event is charged for medication administered during your pet’s stay.
    • Personal belongings will not be excepted, for example, beds, toys, or food dished (except for slow feeders). We will provide your pet with bedding and food dishes.
    • Grooming and bathing are available upon requests. In the event you pet soils themselves during thier stay, a bath will be given at an additional cost. Any dogs boarding more than three days will be required to have an exit bath, additional cost will apply. 
    • All feeding instructions, medication instructions, and emergency contacts will be notated on your pet's account and will be used for future boarding reservations. Please notify staff if any changes need to be made.
    • All pets are boarded or are otherwise handled or cared for by BPAH without liability on BPAH’s part for loss or damage from disease, theft, fire, death, running away, injury, or harm to persons, other pets, or property by said pet, or other unavoidable causes, due diligence and care having been exercised. I shall not hold the hospital or the staff responsible and/or liable in the absence of gross negligence.
  • have read the above pages and understand my responsibilities herein regarding this contract and BPAH’s policies, so the BPAH knows what to expect of me and I know what to expect of the BPAH. I have also read, understand, and initialed the spaces required of me throughout this entire contract.
