  • REQUEST APPROVAL

    Use this option to request approval to add a position, or fill a vacant position.

    • New Faculty positions

    • Requesting permission to refill vacant staff position

     

    HIRE

    Use this option to initiate employment offer process for external/new candidates. This will capture relevant information for offer letter and onboarding set up.

    CHANGE

    Use this option to officially request change for a current employee (ex. Promotion, change in title, special pay, etc).

    Use this option to notify HR and administrative offices of an exiting employee.

If a new adjunct will be participating in any training exercises before start of job role, please add the training start date to ensure accounts are created and ready in time.

    Employee Exit Information

  • This form must be completed by supervisors and managers to provide details related to an employee exit. There are many employee access rights that must be deactivated in order to protect the business of the University. Completing this form alerts our administrative offices that manage these access rights and University-owned equipment. Please provide the following information so that instructions pertaining to the termination of employment may be passed along to affected offices.

    • IMPORTANT NOTES TO SUPERVISOR:

      • Accounts will be disabled at end of business hours of last day of employment, as indicated above. The employee will lose access to email and network at this time. For special circumstances where access to accounts or buildings will be different from the last day of employment, please indicate in the "Other Concerns or Needs" field below to be reviewed by HR and IT.
      • Auto-responder provided below will be set on the exiting employee's email account.
      • Account will be terminated and removed permanently after 30 days.
      • The employee's email account can be shared to supervisor or manager indicated below.
    • Computers and equipment should be turned in to HR on the last day before the employee leaves campus.

    • SOCIAL MEDIA ACCOUNTS

      If your office manages official Greenville University social media accounts, you will want to ensure access is removed, or passwords are changed to accounts to protect content and prevent unauthorized access

       

      BUILDING AND OFFICE KEYS

      All building and office keys should be routed to HR so that inventory and assignments can be properly updated by Facilities.
