    - DTI/SEC Registration (file name format: -DTI)
    - Valid DOT Accreditation Certificate (file name format: -DOT)
    - One(1) 2x2 photo (file name format: -photo)
    - Company profile or overview (file name format: -profile)
    - Photo of the facade of the establishment (file name format: -businessphoto)

    Annual Dues shall be settled upon approval of the application.
