PHOTO CONSENT
I understand that my child may be photographed during AWANA Club and related activities. I agree to allow my child's photo, video, or film likeness to be used for any legitimate purpose by First Baptist Church. When an identification is made, only the first name of the child may be used along with the name of the church.
MEDICAL CONSENT
I hearby consent that my child may receive medical treatment under the direction of any licensed physician in the event of an injury or medical emergency. The authority is granted ONLY after reasonable effort has been made to contact a parent/guardian listed on the registration form. While I understand accidents happen, I will not hold First Baptist Church Bellevue, FBC staff members, and/or FBC adult volunteers responsible in the event my child is injured while participating at AWANA Club. I further understand that I will be held responsible for any payment required for medical or emergency room treatment.
