Lash Integrity Your natural lash health is our top priority! This means that we will only place extensions that are the appropriate weight and length for your lashes and aesthetically pleasing for your bone structure. 

Booking Policy After submitting this form, a direct link will take you to our scheduling app where you'll be able to make your booking. This booking slot is reserved. After the booking is made, you'll need to eft the full amount to secure the booking. Once POP is received the slot will be taken off the availablity sheet.

Arrival Please arrive 15 minutes before or at the time of your scheduled appointment time with clean/makeup free eyes and lashes. 

Cancellation / Rescedule Policy Your appointments are very important to Eyeconic, it is reserved especially for you, we understand that sometimes schedules adjustments are necessary; therefore, we respectfully request at least 48 hours notice for cancellations (or to reschedule). If you need to reschedule before 48 hours of your appointment, you'll be able to apply your payment to your new scheduled appointment. Clients that provide less than 48 hours notice will forfeit 50% of the appointment.

Late Policy Please let us know in advance if you are running late. We will do our best to accommodate you, however, time may not permit us to honor your full service and your appointment may be shortened. We still need to finish your appointment at the scheduled time. 1hour for infills and 1.5hours for full sets from the appointment time. 

No Show Please be courteous and notify us if you cannot make your appointment. Full payment of service will be charged.

Parking Parking is available in front of the location.

Age Policy  Clients must be at least 15 years or older to receive services. Minors under 18 must be accompanied by an adult. 

Refunds + 3 Day Retention Rule   There are no refunds on any services. If you are having any issues after your service with retention, please let us know within 3 DAYS. We will make any changes for free as long as you come see us within seven days from your appointment (we can work around hours). This is your window!

Infills Due to natural shedding process, fills are required every 2-3 weeks to maintain a full look. Anything after 4 weeks is a FULL SET. 40% of the extensions should be in to classify as a infill. Foreign infills (work from another lash tech) will need to be booked in for a removal and a new set. Bookings made for infills will require full payment. 

Aftercare Wash your lashes daily with a lash cleanser or a water based cleanser to prevent infection and keep the lashes healthy and clean. Cleansers can be purchased through Eyeconic at an additional fee. 

Waiver of Liability 

If I experience any irritation, redness, puffiness, itchiness, an allergic reaction or any other side effect of this procedure during or after application, I will use eye drops however if reaction is not cleared within 2 days I will contact a medical doctor immediately. 

There is no guarantee for the bonding time of the eyelash extensions. I understand that there are many factors that may affect the life of the eyelash extensions such as water and moisture contact, weather conditions, and activities involving exposure to high temperatures.

I also agree to defend, indemnify and hold harmless the eyelash extension artist from any and all claims, actions, expenses, damages and liabilities, including reasonable attorneys’ fees which might be asserted against her as a result of my having this procedure performed, or my purchase of these eyelash extension products from her.
