How does this form work?
This form is designed to collect nomination information from multiple individuals in a streamlined process. One individual will start the nomination and then additional contributions can be completed.
- The first nominator will start the nomination form with the nominee's information, their personal nomination, and the contact information of additional contributors.
- Once submitted, two actions will occur:
- The initial nominator will receive a confirmation email with a link to edit the form. The link can be copied and shared with additional nominators.
- Anyone listed as an additional nominator in the initial submission will receive an email to update the nomination form with an edit link.
- Additional nominators will follow the link and add their own details to the nomination. Be sure to not overwrite someone else's submission,
- All nominators have until February 11, 2024 to add, edit, and complete the nomination.
