Registration Form

Preference for Program Enrollment: please indicate 1st and 2nd choices.  If the 1st class choice is full, the child will be placed on a waiting list, but will be enrolled in the 2nd choice class until a determination is made on the class assignment.

**It is the goal of North East Preschool to coordinate and facilitate services from outside support agencies for families as needed. Listing any existing relationships will aid in coordination established services

Required Immunization Records: 

***Please note that immunization records are not necessary to complete registration.  Records may be turned in to the child’s teacher at parent night in August.  If you choose not to immunize your child, please indicate that on this form

As a private program, The North East Preschool operational budget is based on tuition payments.  To continue the quality program, it is imperative that all tuition payments are made in a timely manner.  For this reason, The North East Preschool implements the following policies:

Registration Fee

A $50 non-refundable registration fee, which holds your child's positon in a class and pays for start up supplies, is payable upon registration.  If plans change, and your child will not be attending North East Preschool, please notify the Director at 814-722-7539.  Most often, there is a waiting list and the position may be offered to another child prior to classes beginning in the fall.  The North East Preschool will retain the full amount of the registration fee.

If the registration fee is paid and the preschool cannot accommodate the child due to low enrollment, the registration fee may be returned to the family.

If the full yearly tuition is paid in full upon registering, the registration fee will be waived.

Tuition Payments

Tuition Payments are due on the first of each month from July through March.  If payment deadline are not met and you have made no effort to obtain different payment arrangements with the Director by the 15th of the month, you will be notified of payment due.  If, by the end of month, no payment has been received, The North East Preschool will instate further action.  This may result in a temporary dismissal of your child until payment is received.  It may also results in a permanent dismissal of your child upon receiving no payment at all.

Tuition payments are broken down into monthly installments for your convenience.  The first two payments are due July 1 and August 1.  The first two installments must be made by August 15 (or other arrangements made) in order for your child to begin school.  If payment is not made, or other arrangements have not been made with the Director, the child's positon will not be guaranteed in the class.  If your child is removed from the roster and the position given to another child, The North East Preschool will retain the full registration fee.

If two consecutive payments are missed during the school year and no arrangements have been made with the Director, the North East Preschool Board reserves the right to remove your child from school until payments are received.  This may also results in a permanent dismissal of your child if multiple payments are missed throughout the school year.

If the payment schedule is too difficult to meet, you may contact the Director to arrange a payment plan that accommodates your needs.  However, if terms of the new arrangement are not met, The North East Preschool Board will instate further action.  This could result in a temporary dismissal of your child until payments are received or permanent dismissal of your child if no payments are received.

Scholarships

Scholarships are available to those in financial need.  An application may be obtained from the Director.  After review by the Director, The North East Preschool Board will determine if qualifications have been met and the amount of the award.  Families applying for scholarships are required to make the equivalent of four full monthly payments before awards will be credited to their account.

Withdrawal

If you withdraw your child from North East Preschool any time during the school year, a refund of any advance payment will be prorated and approved by the Director and Preschool Board.  The amount of the refund will be prorated on the basis of the daily tuition rate, an appropriate reimbursement for school supplies, and the date of an official notification of withdrawal.

If the withdrawal is due to non-payment of tuition, no money will be refunded.  All payment due to The North East Preschol prior to the removal of the child will then be due immediately.

Admission Requirements

Children must be the appropriate age for class enrollment prior to May 31st of the program year if he/she will attend North East School District.  Children who attend another school district must meet age requirements of their home districts.  Children must also be toilet-trained prior to the beginning of the school year.
