I acknowledge that I have read and understand the sexual harassment and sexual abuse policies enforced by Gold’s Gym Newburgh Middletown. I understand that Gold’s Gym will not tolerate any employee who commits sexual harassment or sexual abuse and that disciplinary action will be taken against those who are found to have committed these acts, up to and including termination.

I understand it is my responsibility to abide by all the rules contained in the above policies including retaliation against any employee who exercises his or her rights under the policies. I also understand the procedures to report incidents of sexual harassment or sexual abuse as outlined by Gold’s Gym.
