I understand, should I add members during the course of the year (ex. second semester CTE enrollment) and I advance into the next tier of membership, I will be billed for the difference as a separate invoice.I am acknowledging that our FBLA Chapter is committed to Affiliation Dues for the current school year. There will be no refunds given for membership after the submission of this form. I acknowledge that my chapter and school are responsible for any associated fees for the Program Affiliation Model.Program Affiliation is an allowable expense under Perkins V in Iowa. Affiliate 100% of studentsin the entire program of study including both semesters of Carnegie units, MOC/work-basedlearning, and internship experiences.
