In consideration of the permission granted by the Guam Football Association (GFA) and/ or its affiliate organizations to utilize all or part of the GFA Facility, I, as the duly authorized representative of the Team/Club/Association and on behalf of all the Team/Club/Association participants, hereby release and discharge GFA, its clubs, representatives, officials, coaches, agents, employees, officers, successors, sponsors and assigns, from all claims, demands, actions, judgments, and executions, which the undersigned ever had, now has, or may have, for which the undersigned heirs, executors, administrators, or assigns, may have or claim to have against GFA, its clubs, representatives, officials, coaches, agents, employees, officers, successors, sponsors, and assigns, for all personal injuries, whether known or unknown, caused by, or arising out of, the usage of the GFA facility. Additionally, we agree to comply with all rules that GFA has posted in the facility and set forth, but not limited to, placing all waste in receptacles, not erecting canopies or tents on or near the area being used in the facility.

GFA reserves the right to reschedule or cancel reservations with multiple dates due to unforseen conflicts and will provide ample notice via the contact information provided.
