    • No medication will be given to a child without written consent from a parent or carer of the child concerned.  Any medicine must be prescribed by a medical Doctor and have your child's name on the label.  You must also complete a Medicine Consent Form to authorise us to give the medicine to your child each day if it is necessary.

    • I agree that in the event of an emergency, my child can be seen by a Doctor or healthcare professional or taken to hospital should the need arise.

    • Nursery Opening and Closing Times

    • I am aware that Hopscotch opens at 7.30am and closes at 6pm.  I agree to use the service only between those times.  I also agree that if I am going to be late I will contact the nursery as soon as possible and make other arrangements for the ccollection of my child.  I am aware that late collection will incur a fee of £10 for each 15 minutes past 6pm.  

      I understand the importance of these times being adhered to due to insurance cover and strict staff ratios which must be upheld by the nursery.

    • Hopscotch Day Nursery has a Facebook page which shows pictures of our rooms, some artwork and upcoming events.  We also sometimes add photos of the children taking part in various activities inside and outside the Nursery.

    • Mobile Phones & Cameras

    • I am aware that Hopscotch has a policy on mobile phones and cameras not being used in the nursery.  I agree not to use my mobile phone or camera whiilst in the vicinity of the Nursery.  If I need to use my phone, I will seek permission from the Nursery Manager or a senior member of staff before doing so. 

    • Outdoor Activities

    • Hopscotch Day Nursery encourages the children’s experiences to extend beyond the classroom and would like to take the children out as much as possible. 

       

    • Do you consent to your child using the nursery gardens, school grounds and visiting local places of interest within a mile radius of the nursery?*
    • Do you give your consent for Hopscotch staff to administer Calpol or Neurofen if your child has a temperature over 100? The Nursery will ring you and if the temperature doesn't go down you will be asked to collect your child. They will not be allowed to return to nursery until they have been clear of all symptoms for at least 24 hours.*
    • Do you give consent for staff to use Baby Wipes and Sudocrem?*
    • Do you give your consent for Hopscotch staff to administer basic first aid (i.e. application of cold compress) or the use of any contents of the first aid boxes such as plasters or antiseptic cream, located on site or CPR (all of which will be carried out by trained staff) if and when necessary?*
    • Do you give your consent for Hopscotch staff to administer antihistamine (allergy relief) if your child appears to have had an allergic reaction to a sting or bite etc?*
    • Do you give your consent for Hopscotch staff to apply sun block protection cream to your child as a top up when necessary?*
    • Do you consent to photographs of your child being used on wall displays in the nursery?*
    • Do you consent to photographs of your child being used for events (ie plays, concerts and fund raising) that may appear in local newspapers or other local publications?*
    • Do you give permission for your child's photo to be used on our Facebook and Twitter pages?*
    • Do you give permission for your child to be observed and have their photograph used for their own and other children's learning journals and for students to use their photos to form part of their coursework?*
    • Do you give permission for your child to have their face painted on special occasions?*

    • I understand that if my child has sickness or diarrhoea they will not be allowed to return to nursery until they have been clear of symptoms for at least 48 hours.

      I understand that if my child has been prescribed antibiotics or any ther medication, they must stay away from Nursery for the first 48 hours due to the possibility of side effects or reactions to the medication.
