STATEMENT OF RESPONSIBILITY As sponsors of this activity, I (we) agree to abide by St. Padre Pio parish policies and procedures that apply to use of this space. We assume the responsibility of clean up and guarantee payment for any damages. We understand that no food or beverages are allowed in classrooms 4, 5, 6, 7, 8, 9.
Shrine of St Padre Pio Facility Rules
1. A request for use of facility must be filled out by the the person responsible for upholding the regulations for usage of the room(s).
2. The person making the request must read and sign the agreement.
3. There is a picture on the inside window with the layout sketch indicating how the room MUST be left before you depart the facility.
4. All chairs are to be stored in stacks of 10 and placed in the alcove area shown in the layout plan.
5. All trash must be tied and taken to the dumpster. A please obtain the code from the parish office ahead of your use.
6. If you are in the kitchen, and will be serving refreshments, you are responsible for sweeping the floor (broom and dustpan are available). If food/drink is on the floor, please use the mop and bucket provided to clean the area.
7. Pick up all things that might have fallen on the floor during your occupancy.
8. If white board is used, please erase.
9. Make sure the doors are closed upon departing.
10. If you are the last person leaving the facility, you will be responsible for assuring that the exterior doors are locked. You are responsible for arming the building. If you need assistance in this process, please contact the parish office ahead of your meeting, during our work day and we will assist you.
