In consideration of my child's participation, I agree to indemnify St. Jude of the Lake Catholic Church and the Archdiocese of St. Paul & Minneapolis from any claims or law suits brought against St. Jude of the Lake Catholic Church/Archdiocese of St. Paul & Minneapolis by myself, my child or others, that arise out of any behavior by my child at the event/activity/class session described above. I also agree to pay reasonable attorney's fees or expenses incurred by the St. Jude of the Lake Catholic Church/Archdiocese of St. Paul & Minneapolis in defense of such a claim/suit.

In case of emergency, I understand that every effort will be made to contact me. If I cannot be reached I hereby give St. Jude of the Lake Catholic Church the permission to act in my behalf in seeking emergency treatment for my child in the event that such treatment is deemed necessary by St. Jude of the Lake Catholic Church.

I give my permission to those administering emergency treatment to do so, using those measures deemed necessary. I absolve St. Jude of the Lake Catholic Church from liability in acting on my behalf in this regard so long as St. Jude of the Lake Catholic Church is not grossly negligent. 2026-27
1. St. Jude’s primary curriculum is a combination of self-made, YDisciple, Theology of the Body, and other relivant resources. St. Jude focuses on the Holy Spirit, liturgical year, sacraments of initaion, and Catholic teachings. Most of our classes are done in a large group format, with the last 10-20 minutes in small groups.

3. St. John Paul II’s Theology of the Body offers a very positive message of the gift of sexuality as a good and essential part of love and life. It is our hope that this curriculum will provide your child with the knowledge and understanding they need concerning the meaning and purpose of their identity, their sexuality, and of their very existence. I expect your Confirmand to approach these lessons with respect and maturity. Communication/Fee/Registration

Please read carefully
Flocknote, an email, and a texting app will be used for all Faith Formation communication and updates. Registration for Flocknote is a mandatory part of Faith Formation. Simply text ‘SJOTL’ to 84576 to sign up. You may also register online at https://stjudeofthelake.flocknote.com/

A weekly flocknote will be sent out, typically on Tuesday's. Please pay attention to these and complete any simple tasks that may be within them.

Administrative Fee
Our 8th Grade Confirmation program cost is all-inclusive (programming and retreat): $250* *Notes: (1) Financial aid is available in exchange for volunteer service. (2) This fee includes our St. Jude Confirmation Retreat. If you are unable to commit to the retreat and need to attend an alternate NET Confirmation retreat, you are responsible for the cost.

In order to complete the registration process, at least one parent MUST attend the Parent Meeting on Wednesday, September 23th in the church. You will recive most information at the meeting and via Flocknote!
Confirmation sessions are from 4:15 to 5:30 on Wednesdays.

After reviewing the Calendar, by signing you agree for your child to attend Wednesday sessions with the exception of up to 3 excused absences. If your child is misses a class, you agree to filling out the absence form. Missing more than three sessions may result in delaying your child in receiving the Sacrament of Confirmation.

St. Jude of the Lake would love to share the joy and excitement of our Confirmation journey as we prepare for the Sacrament! With your permission, photos and videos of your child may be used in parish communications, social media, newsletters, and promotional materials to celebrate and share the faith-filled experiences of our Confirmation program.
I, the undersigned parent/guardian, give permission for my teen to be photographed and/or recorded during program activities. I understand that these photos and videos may be used by St. Jude of the Lake for promotional purposes, including posting on Instagram and other social platforms. I acknowledge that: My teen’s image may be shared publicly. No sensitive personal information will be included. This permission is voluntary and can be withdrawn at any time by contacting the Youth Minister, Elizabeth Cairns via email at ecairns@stjudeofthelake.org.
