• Parent/Guardian Tutoring Agreement

  • This agreement in and of itself is intended to acknowledge terms and services provided. This agreement is not a purchase of service nor does it require that a purchase of service be made.

  • Services Provided

    Tutoring services provided by Reilly Math Tutoring LLC are defined as tutoring sessions. Tutoring sessions are determined by subject and duration. The subject, duration, and price of each tutoring session are stated on each service type on the Reillymathtutoring.com scheduler. All services to be purchased are selected by the customer/user of Reillymathtutoring.com.


    Reilly Math Tutoring LLC is not responsible for outcomes in any form, including but not limited to, grades, test scores, or acceptance to any institution as a result of services provided.

    Reilly Math Tutoring LLC reserves the right to refuse or terminate service to anyone for any reason.

     

  • Payment for Services

    Customers of Reilly Math Tutoring LLC must provide payment information in the form of a credit or debit card at the time of scheduling an initial tutoring session.

    By signing this agreement you, the Parent/Guardian or student of at least 18 years of age, authorize Reilly Math Tutoring LLC to charge your credit or debit card for agreed upon purchases of tutoring sessions. You acknowledge that your information will be saved to file for future transactions on your account.

    Customers have the option to pay for tutoring sessions in advance at the time of scheduling. If the session is not paid for in advance, the customer will be charged the price of the session immediately upon completion of the tutoring session.

     

  • Cancellation/Refund Policy

    If you need to cancel or reschedule a tutoring session, you must do so at least 24 hours before the scheduled time to avoid a cancellation fee.

    If a session is canceled less than 24 hours before the scheduled time, a cancellation fee of 50% of the cost of the session will be applied.

    If the session was paid for in advance and is subject to the cancellation fee, a refund of 50% of the cost of the session will be given.
    If the session was not paid for in advance, 50% of the cost of the session will be charged. 

    If you would like to reschedule a session, you must select an alternative time slot that is within 2 days of the originally scheduled time to avoid the cancellation fee.

    If an alternative time is not selected or not available within 2 days of the original time, then the cancellation fee will be applied. For extenuating and/or emergency circumstances, please contact Reilly Math Tutoring to discuss options.

    Following three cancellations without proper notice, all subsequent cancellations without proper notice will be charged for the total cost of the session.

  • Terms and Conditions

    By completing this form and utilizing Reilly Math Tutoring you agree to the Terms and Conditions listed on Reillymathtutoring.com. The Terms and Conditions document can be found at:

  • Consent to Tutoring Services

    By signing below you, the Parent/Guardian or student of at least 18 years of age, acknowledge and give consent to Reilly Math Tutoring LLC to conduct virtual, online tutoring sessions with the above named student during the scheduled session time. Parents/Guardians may be present with their student during the tutoring session at any time and may observe the session at any time. The Parent/Guardian may revoke this consent by issuing a written request to terminate this agreement via email communication to connect@reillymathtutoring.com. The request to terminate this agreement will be effective upon receipt of the email containing the request. Reilly Math Tutoring may also terminate the agreement with email notification to the Parent/Guardian.
